Patient Summary
Definition:

A Patient Summary (PS) is a concise clinical document that provides an electronic  patient health data set applicable both for unexpected, as well as expected, health care contact.

A PS provides an HCP with essential information needed for health care coordination and, in case of an unexpected need, for the continuity of care, or when the patient consults an HCP other than his regular contact person (e.g. the general practitioner he/she is registered with).

The epSOS PS does not include a detailed medical history, details on clinical conditions or a full list of all prescriptions and dispensed medicines.
